Accu Reference provides supplies for collecting specimens to our clients. Supplies can be ordered in 3 ways:

• Use the Online Inventory Order Form
• Print out the Supplies Request Form and fax the complement form to 908-474-0032
• Send Supplies Request Form to customer service

Accu Reference will only supply items that are permitted in accordance with the laws of your jurisdiction, and these may vary from state to state.
